NHCP Program

Overview
The Clinical Programs Assistant provides administrative support the the Neighborhood Housing & Care Program clinical team. This is a part time position, 24 hours a week.
General Information
Job Duties:
  • Perform administrative duties for the NHCP team
  • Client Management System (CMS) and U:drive client chart maintenance (updating Face Sheets, tracking ROI’s, inputting data etc.)
  • Triage calls/requests and coordinate with NHCP staff members to ensure that request is addressed
  • Contact outside agencies to send and receive chart notes and paperwork
  • Act as liaison to collaborators and other providers (case managers, clinics, etc.) as needed
  • Other administrative duties as required
  • Work with NHCP Interim Director to identify program needs and create and implement new processes
  • Input referral data into CMS, assist with updating the waitlist as needed
  • Assist with housing searches and case management as needed
  • Gather information, create and distribute monthly resident/client newsletter
  • Assist with special projects related to strategic program planning
  • Work with IT department on special projects including basic IT support to staff
  • Coordinate agenda and Outlook invites for program meetings
  • Initiate field safety protocol with each clinician at the end of the workday
  • Acquire other necessary materials as needed such as OT supplies, emergency food and holiday items
  • Assist OT Director with special events as needed
  • Summarize and document pertinent clinical information in weekly team meetings
  • Manage monthly client insurance spreadsheets
  • Organize the distribution of client’s holiday donations
Qualifications
Qualifications:
  • College Degree or equivalent
  • Strong computer skills including Word, Excel and Outlook
  • Experience with health systems and Electronic Medical Records (EMRs) preferred
  • Oregon driver’s license
  • Strong organizational skills and ability to multi-task
  • Ability to take initiative and strong attention to detail
  • Effective, responsive and respectful communicator
  • Proactive problem solver
  • Willing to work autonomously as well as with an interdisciplinary clinical team
