Social Worker, full time
NHCP Program

We are seeking a full time social worker to join our Neighborhood Housing and Care Program (NHCP) team. This position is part of a multi-discipline team that includes Registered Nurses and Occupational Therapists to provide in-home, client-centered, team-oriented services to inspire people living with HIV to live well. Our philosophy of care includes using harm reduction, non-violent communication, trauma informed care and self-determination.

Social Workers are responsible for developing and implementing creative, clinically appropriate service plans using client-centered approach, case management, assisting with crisis situations as needed, providing routine psycho-social evaluation, administering mental health screening tools and facilitating mental health/addiction groups as appropriate.

  1. Masters in Social Work, at least 2 years clinical experience preferred working with persons living with complex mental health and substance use issues.
  2. Ability to assess and implement service plans and clinical interventions related to mental health/other psycho-social issues of persons with HIV and/or chronic illness, in culturally appropriate manner.
  3. Experience with interdisciplinary team function and strong commitment to collaboration.
  4. Efficiently manage time and caseload. Able to work autonomously, self-initiate and be dependable with task completion, and adhere to documentation and service plan timelines.
  5. Understanding and familiarity with community resources.
  6. Verbal and written communication skills, including principles of clinical documentation.
  7. Ability to use current technology, effectively use mobile communication devices.
  8. Competency in facilitating process-oriented and psycho-educational groups.
  9. Working knowledge of Harm Reduction and Compassionate Communication (NVC) models.
  10. Working knowledge of current Social Work theory and practice and associated ethical and legal issues.
  11. Current CPR certification, or willingness to be certified within 90 days of hire.
  12. Valid driver’s license, access to a personal vehicle and auto insurance that meets minimum Our House requirements.
