The Neighborhood Housing and Care Program (NHCP) is an innovative program that offers permanent housing plus supportive services for people living with HIV/AIDS. NHCP is able to provide this scope of services through collaboration with Cascade AIDS Project (CAP) and the Partnership Project (OHSU). Our House provides in-home services such as nursing, social work, and occupational therapy as needed to support independent living, while CAP provides services to assist with obtaining affordable, permanent housing. The Partnership Project provides some of the Nursing care. NHCP advocates and works with clients to maintain optimal health, live in safe and affordable housing, in order to remain as independent as possible.
NHCP is designed to provide services in each client's home. At the time of referral, the client will be assessed for services. If it is found that the client is eligible for services and does not have stable housing the client will be to referred to an agency that will assist in acquiring stable housing. Services through NHCP will begin at the time stable housing is obtained.
The Neighborhood Housing and Care Program is the recipient of the 2010 Innovative Practices Award from the Oregon Department of Seniors and People with Disabilities under DHS.
The Neighborhood Housing and Care Program (NHCP) was introduced in 2004. NHCP provides Nursing Care, Occupational Therapy, and Social Work services to those who want to live independently in the community, but still need a moderate level of support. We work with Cascade AIDS Project to provide rental assistance when needed, and the Partnership Project who helps with nursing care.
