NHCP Exclusion Criteria
The following criteria will be considered when assessing a client for continuation in NHCP
MEDICAL
- Inability to learn and manage complex medication.
- An unstable condition or symptoms that require continuous monitoring.
- Evidence of an inability to manage symptoms (fatigue, nausea, etc) that may lead or have led to repeated hospitalizations/ED visits.
- Change in condition resulting in deteriorating ADL abilities that require assistance that is currently not available.
- Have severe dementia or behavioral issues that contribute to the management of any of the above issues
MH/CD
- Inconsistent adherence with treatment resulting in a significant impairment to the ability to maintain adequate daily functioning.
LEGAL / SAFETY
- Inability to demonstrate a safety of or toward others: Including a history of violent or sexually predatory behavior within the past 12 months of independent living.
- Current required mandated reporting as a registered sex offender.
- Unresolved felony criminal activity including: outstanding bench warrants, pending charges, unserved sentences.
