NHCP Inclusion Criteria
- Independence with mobility, with or without adaptive equipment, to perform functional tasks.
- Adequately perform the majority of the following ADL?s (Activities of Daily Living), with or without adaptive equipment or available assistance.
- Dressing, bathing, toileting, grooming
- Food preparation, meal planning, cooking
- Eating/feeding
- Scheduling and keeping appointments
- Maintenance of home environment
- Ability to get food and water (able to identify and demonstrate basic nutritional skills)
- Ability to demonstrate or verbalize safe behaviors:
- Smoking
- Food handling
- Recognition of hazard awareness (fire safety, fall potential)
- Ability to access emergency services
- Adequately manage finances, with or without a representative payee or other assistance.
- Must be under the care of a Primary Medical Provider.
- Must sign a Release of Information form in order for NHCP staff to speak with appropriate service providers, in order to assist with continuity of care.
- Must be willing to participate in the initial assessment process with the NHCP staff.
- Must not possess illegal drugs or weapons on the premises.
