NHCP Inclusion Criteria

  1. Independence with mobility, with or without adaptive equipment, to perform functional tasks.
  2. Adequately perform the majority of the following ADL?s (Activities of Daily Living), with or without adaptive equipment or available assistance.
    • Dressing, bathing, toileting, grooming
    • Food preparation, meal planning, cooking
    • Eating/feeding
    • Scheduling and keeping appointments
    • Maintenance of home environment
    • Ability to get food and water (able to identify and demonstrate basic nutritional skills)
  3. Ability to demonstrate or verbalize safe behaviors:
    • Smoking
    • Food handling
    • Recognition of hazard awareness (fire safety, fall potential)
    • Ability to access emergency services
  4. Adequately manage finances, with or without a representative payee or other assistance.
  5. Must be under the care of a Primary Medical Provider.
  6. Must sign a Release of Information form in order for NHCP staff to speak with appropriate service providers, in order to assist with continuity of care.
  7. Must be willing to participate in the initial assessment process with the NHCP staff.
  8. Must not possess illegal drugs or weapons on the premises.
